I have tried to bake bread in my bread machine but the last couple of times the top collapsed. :( I'm not sure what I am doing wrong, I have tried both the box mix from the store and from scratch. I am using bread flour and yeast for bread machines but the same results. It tastes okay but sure isn't looking right! Any suggestions?

I had something similar happen yesterday. I've baked bread off and on over the years but have only been doing it regularly for about a month. This is the first time it's happened to me... ever. The top looks shriveled, not totally flat on mine. In my case I think the dough was too moist. I noticed it was really sticky when I was kneading it. I just couldn't get it to the right consistency. I've been experimenting with mixing white and wheat flour so I can slowly transition my family to wheat.
I'm not sure but I suspect one of three problems with mine:
Bad Flour mixture
bad humidity - not sure if it's too much or too little. Yesterday was the warmest driest day I've baked yet (in the 60's, sunny)
Not baking long enough

In your case my first suspect would be the machine since it seems to be the only constant in the process. (Unless you're adding your own yeast to the boxed mix in which case I'd try testing that to see if your yeast is dead/dying)
