Been seeing a lot more adapters for shooting .22lr in .45's, .32 in .308, 30-06, and today I saw one to shoot .410 out of a .12 ga. in the Firequest cat.

Can't see where they would be very accurate.
Anyone tried them?

I have numerous adapters and insert bbls., and it depends on what your definition of "accurate" is?
Will they group well? Well some of them do. Will they shoot to the sights? Not likely...
Now, the Krieghoff insert bbls that i have? They ALWAYS shoot to the sights, and shoot as well as a decent bolt action 22! They are top of the line, and "yes" these days they are expensive...

There was an artical in Shotgun news a few months ago about a guy buying some inserts for an Lee-Enfield and a Springfield 1903, the adapter let the person shoot a .32acp. When he shot the .32apc out of the Springfield it didnt make it out of the barrel. He shot the Enfield and said it worked best at 50 meters.
As far as shooting .22 out of a 12gauge with an adapter there was a great artical in The Backwoodsmans magazine a few years ago about them. Theres only so much acurracy you can get out of throwing a .22 down a shotgun barrel, on top of the insert being 3 or 4 inches long....I still look for them at gun shows though..

I've fired quite a few acp's using adapters, i've yet to have any problem with them not fireing or the bullet not leaveing the bbl...
As for the 22lr's in a 12ga... Why would it matter to the accuracy that the bullet is coming out of a shotgun bbl.??? As he bullet doesn't touch the shot bbl.! Have you ever shot a stubbie pistol?? I've shot a huge amount of grouse with a 22lr Raven pocket pistol, they are VERY accurate and have what? Maybe an 1-1/2" bbl?.
The problem is, the adapter can move around in the shotgun chamber, and you don't always put it in in the EXACT same way/place, so it doesn't group well. I have one of them around here too...someplace?
